Abstract

Although it is impossible at this time to reach a conclusion as to the role of conservative surgery in the treatment of breast cancer, it appears evident that that procedure will occupy an important space in the future strategy of breast cancer management. There is an increasing number of protocols, in various institutions, where limited surgery is the treatment of choice for certain subgroups of breast cancer patients. I believe that it is improper to try to solve the problem of conservative versus extended surgery in breast cancer in terms of 'either/or' decisions, and that both approaches may find their place in a modulated program of treatment of breast cancer. Such a program would consider different procedures according to the size and the site of tumor, as well as the clinical and pathologic condition of regional lymph nodes in different subsets of cases. Future clinical research will provide new data for continuous adjustment of the present plans, hopefully in the direction of higher cure rates and better quality of life for the treated patients.
