Abstract
Forty-six patients with advanced prostate cancer and bladder outlet obstruction, unrelieved by total androgen ablation, were submitted to transrectal microwave hyperthermia of the prostate. Twenty-four patients had an indwelling catheter and 22 were obstructed but not in urinary retention. Hyperthermia was administered in ten, sixty-minute long sessions, with a calculated intraprostatic temperature of 43 ± 0.5°C. At one year follow-up. 12 patients (50%) were still catheter-free. Mean residual urine volume was significantly reduced (p <0.05), while peak flow rate and maximum flow nomogram were improved but not significantly. Most patients experienced a marked improvement of subjective symptoms and quality of life. The only major complication was a prostatorectal fistula that was cured by leaving a urethral catheter in place for one month. Transrectal prostatic hyperthermia is a safe and effective palliative procedure for bladder outlet obstruction caused by advanced prostate cancer.
