Abstract
Infection by Rhodococcus equi represents a rare but already documented event in HIV-infected individuals, leading to the development of chronic granulomatous lesions in lungs and in other organs as well; these lesions respond only partially to antimicrobials. We recently diagnosed (through specimens obtained with BAL and/or transbronchial biopsy in all cases and through blood cultures in 2 pts.) 4 cases of pulmonary rhodococcosis in 4 males with a mean age of 29.7 yrs. (26 to 34) and a mean CD4+ lymphocyte count of 53/mcL (19 to 106). Lung involvement was localized at lower right lobe and middle right lobe in 1 case each, while the 2 remaining pts. showed a lesion at the upper right lobe. Combinations of 2 or more antibiotics were administered in all cases and in 2 pts. a surgical intervention was performed, without any major post-surgical complications. Our experience seems to point out that the surgical ablation of chronic inflammatory lesions caused by R. equi in lungs should be evaluated even in AIDS pts.
