Su di un case di ricostruzione del dorso della mano con lembo venoso a peduncolo distale

Reconstructive surgery of the upper limbs comprehends a variety of different procedures, from skin grafts to the use of microsurgical free flaps. To obtain satisfactory results, the indication to use a simple method versus a complicated one varies according to three different criteria: anatomical, functional and aesthetic. In this paper, we present the case of a female patient who had already undergone surgical removal of a melanoma of the hand dorsum and needed further excision and plastic reconstruction. The hystology of the tumor required total excision of the skin involved, 3 cm wider than the previous scar and including the fascial layer. Reconstruction therefore implied the use of a rather large and thick cutaneous flap, with a good blood supply and whose texture was as close as possible to the skin excised. We have therefore used a fasciocutaneous flap from the forearm, with one venous distal pedicle from the collateral venous system of the cephalic vein.
