N2 - A patient with hypertension was admitted for an asymptomatic, pulsatile mass of the right neck detected by a screening duplex ultrasound scan. Contrast-enhanced computed tomography with three-dimensional reconstruction disclosed a 26-mm aneurysm of the right inferior thyroid artery (ITA) associated with a synchronous 10-mm aneurysm of the contralateral ITA. The patient was operated on under general anesthesia through right cervicotomy. The aneurysm of the right ITA was surgically cut, and the right ITA was binded, sparing the recurrent laryngeal nerve. Aneurysms of the ITA are rare but deadly lesions that may clinically appear with hoarseness, dysphagia, and respiratory distress. Contrast-enhanced computed tomography selectively allows a noninvasive and accurate anatomic assessment of both right and left thyrocervical trunks.
